Changes in the Reading Ability of Patients with Accommodative Insufficiency while Wearing a Plus Lens

Abstract
Purpose: The purpose of this study was to compare the change in reading ability in patients with accommodative insufficiency while wearing a plus lens.

Methods: In this study, after refraction and visual examination, 167 participants were classified into the normal group, binocular dysfunction group, and accommodative dysfunction group. We used Visagrap II (Readalyzer) to measure the duration of fixation, number of regressions, saccades in return sweeps, and reading rate. Accommodative insufficiency in the accommodative dysfunction group was examined by testing reading ability while wearing a plus lens.

Results: The study revealed that the fixation duration and reading rate were lower, and the number of regressions, and saccades in return sweeps were higher in the accommodative dysfunction group compared to the normal group; however, these differences were not statistically significant. After wearing plus lenses, patients with accommodative dysfunction showed no difference in saccades in return sweeps, an increase in fixation duration and reading rate, and a decrease in the number of regressions, though these changes were not statistically significant.

Conclusions: It seems that the reading rate is improved due to an increase in fixation duration and a decrease in the number of regressions because of the improvement in near fixation and asthenopia by prescribing plus lenses to patients with accommodative insufficiency. Therefore, it is posited that the prescription of plus lenses is effective as a method to improve the asthenopia caused by accommodative insufficiency. It is necessary to study the symptomatic relief in the accommodative insufficiency through prescription of plus lenses in the future.
